Heart & Grace Consulting provides disability-related psychosocial evaluations for individuals seeking formal documentation to support benefit determinations, workplace or academic accommodations, and access to services. These evaluations are non-therapeutic in nature and are designed to clearly document how medical, mental health, or psychosocial conditions impact an individual’s daily functioning. The purpose of the evaluation is to provide structured, professional information for administrative or legal review, not to offer treatment or ongoing care.

Purpose of Disability Evaluations
A disability evaluation is a structured, objective assessment designed to document an individual’s functional limitations, psychosocial history, and contextual factors relevant to their capacity to work, learn, or independently manage activities of daily living. These evaluations are frequently requested in connection with disability benefit applications or appeals, accommodation requests, and other documentation requirements for administrative, employment, educational, or legal proceedings.

The evaluation prioritizes functional impact over diagnosis alone and results in a comprehensive written report prepared in clear, professional language. Reports are tailored to the stated purpose of the referral and are suitable for review by attorneys, adjudicators, administrative agencies, and other non-clinical decision-makers.
Evaluation Process
Each disability evaluation begins with a screening to confirm that the requested service is appropriate and within scope. Once confirmed, informed consent is obtained and the purpose, limits, and intended use of the evaluation are reviewed in detail. The evaluation itself typically involves one or more structured interview sessions conducted virtually, during which relevant psychosocial history, functional limitations, and contextual factors are explored. When available and appropriate, collateral documentation such as prior evaluations, medical records, or agency correspondence may be reviewed to support the assessment. Following completion of the evaluation sessions, a comprehensive written report is prepared that reflects the information gathered and addresses the stated purpose of the evaluation.

Once the evaluation sessions are completed, reports are typically finalized and delivered within 10 to 15 business days. Expedited timelines may be available for an additional fee, subject to scheduling availability.
All disability evaluations are conducted using a trauma-informed, culturally responsive, and ethically grounded approach. We recognize that disability processes can feel invasive, overwhelming, or high-stakes, and care is taken to ensure that the evaluation process is respectful, transparent, and clearly explained at every stage. All information shared during the evaluation process is handled in accordance with applicable privacy and confidentiality standards. Reports are released only to the individual or authorized recipient as specified in the consent agreement.
Rates & Fees
A standard disability evaluation package is offered at a flat rate of $800, which includes up to two structured interview sessions lasting 60 to 90 minutes each, review of provided collateral documentation, and preparation of a comprehensive written evaluation report.​ More complex evaluations may require additional time due to extensive records review, multiple systems involvement, appeal-related documentation, or additional interview sessions. In these cases, fees typically range from $1,200 to $1,700, and any adjusted rate is discussed and agreed upon in advance of proceeding.

Additional services, when requested, may include:
Extra interview sessions at $250 per 60-minute session,
Expedited report preparation within five business days for $300
Brief report addendums for $200.

No additional services are provided without prior consent. Consultations fees are put towards the cost of services.
