About the Role
The Psychiatric Nurse Practitioner (Per Diem) provides psychiatric evaluation, medication management, and collaborative treatment planning for clients receiving outpatient mental health services through Heart & Grace Therapy, LLC. This role functions as part of an interdisciplinary, trauma-informed team and supports continuity of care for clients receiving psychotherapy services. Services are primarily delivered via telehealth, with flexibility in scheduling based on client and provider availability.
Key Responsibilities:
Conduct comprehensive psychiatric evaluations and diagnostic assessments
Prescribe, manage, and monitor psychotropic medications in accordance with state and federal regulations
Provide ongoing medication management visits, including monitoring effectiveness, side effects, and adherence
Collaborate with therapists and other providers to coordinate integrated treatment plans
Maintain timely, accurate, and compliant clinical documentation within the electronic health record (EHR)
Provide psychoeducation to clients regarding diagnoses, medications, and treatment options
Adhere to ethical standards, scope of practice, and all applicable regulatory and insurance requirements
Participate in case consultations and interdisciplinary communication as needed
Requirements
License: Active PMHNP license in the state(s) of practice (MD, DC, and/or CT as applicable)
Education: Master’s or Doctoral degree in Nursing from an accredited program
Other Requirements:
Current DEA registration and state-controlled substance registration (where applicable)
Board certification as a PMHNP (ANCC or equivalent)
Eligibility for credentialing with commercial insurance plans
Current professional liability insurance
Preferred Skills & Experience
Experience providing care via telehealth platforms
Familiarity with trauma-informed, culturally responsive, and person-centered care models
Experience working with diverse populations and complex clinical presentations
Strong collaboration and communication skills within interdisciplinary teams
Comfort working in a growing private/group practice environment
Experience with EHR systems (TherapyNotes preferred)
Compensation
Per-Session Rate: $150–$200 per completed medication management visit (based on visit length and payer mix)
